• Your credit card number and expiry date
• The type of donation you are making
• The name and address of the family or friend you would like to be notified of this donation (if applicable)

Your income tax receipt will be mailed to you within 4 weeks. If you would prefer to receive an e-tax receipt please let us know.

• Print the Donation Form MS Word, PDF
• Fill in all of the information for your donation
• Please ensure that your name and address are printed clearly
• Mail the form to our office along with your credit card information or a cheque made payable to Alzheimer Society of Toronto (please do not send cash in the mail)
